- ADRC Reports due on the 10th of each month, enter the previous month’s data.
- All core ADRC membership agencies are to submit data each month.
- Options Counseling Semi-Annual (also known as Narrative) Report has been eliminated.
- Options Counseling Consumer Satisfaction surveys are mailed back to the Core ADRC agency that is providing the Options Counseling service.
- Please remember to fill in the back area of the survey to your agency’s information.
Options Counseling Consumer Satisfaction surveys are available in: English, Spanish, Portuguese, Haitian Creole, Chinese-Mandarin (Standard), and Russian. Please reach out to Karyn.Wylie@mass.gov to receive a copy of the survey if needed
